As we continue to grow and expand our services, we are seeking a dedicated and experienced Homecare Supervisor to join our team.
Role Overview:
The Homecare Supervisor will be responsible for overseeing the delivery of homecare services to service users, ensuring that care plans are implemented effectively, and high-quality care is provided. This role requires strong leadership skills, health care provision expertise, and a commitment to service user-centred care.
Key Responsibilities:
Service user assessment:
Conduct initial assessments of service user care needs, risk assessments etc, preferences, and goals in collaboration with service users, families, and healthcare providers. Develop personalised care plans based on assessment findings and update them as needed.
Care coordination:
Coordinate the delivery of homecare services to service users, including liaising with and updating caregivers, assigning tasks, and ensuring that care plans are followed. Monitor service user progress and adjust care plans as necessary to meet changing needs. To coordinate delivery of PPE with care staff when required. Manage day to day queries from staff or service users in relation to care delivery.
Staff supervision:
Supervise and support homecare staff, provide guidance, training, and feedback to ensure that staff members deliver high-quality care and adhere to professional standard. To train new carers in best techniques, methods & resource use in service users' homes. To provide in house carer reviews in the service users' homes. Act as a liaison between carers & head office.
Service user advocacy:
Advocate for service user rights, preferences, and interests within the homecare team and community. Serve as a liaison between service users, families, caregivers, and other stakeholders to address concerns, resolve conflicts, and ensure that service user needs are met effectively. Attend meetings with PHNs, OTs or GPs as requested
Quality assurance:
Implement quality assurance measures to monitor and evaluate the quality of homecare services provided. Conduct regular assessments, audits, and reviews to identify areas for improvement and ensure compliance with regulatory standards and best practices.
Ensure all equipment provided is being used correctly. Report any accidents /incidents or near misses to the QA dept. Complete any other tasks related to the delivery of care as decided by the QA Manger /CEO.
Emergency response:
Develop and implement emergency response protocols to address medical emergencies, accidents, and other unexpected events in service user homes. Train homecare staff on emergency procedures and coordinate with emergency services as needed.
Qualifications:
Minimum required QQI level 6 in healthcare
Preferred bachelor's degree Social Work or Nursing qualification
3+ years of experience in homecare or community-based healthcare, with demonstrated supervisory experience.
Strong leadership, communication, and interpersonal skills.
Strong computer skills, word, excel etc
Knowledge of homecare regulations and best practices.
Empathy, compassion, and a service user-centered approach to care.
