To manage, organise, and co-ordinate the physiotherapy service for patients requiring acute rehabilitation and discharge planning on general medical wards under medical specialist teams including GI, endocrine, renal and infectious diseases [including Covid Pathway & National Isolation Unit].
To complement existing acute rehabilitation and frailty/older persons services to provide standardised high quality physiotherapy across multiple wards and medical teams To seek to continuously optimise care and outcomes for all patients presenting to MMUH, including close working with Inpatient Renal Rehabilitation, Acute Medicine, FIT and SGW services to improve the provision of physiotherapy care for persons admitted acutely to general medical wards.
To provide targeted interventions and case management to persons admitted to MMUH under various medical specialities including GI, endocrine, renal and infectious diseases, including Covid Pathway & National Isolation Unit.
To establish and maintain close collaborative working with other physiotherapy acute rehabilitation teams.
To commence discharge planning within an appropriate timeframe and to facilitate timely, safe and weekend appropriate discharge of patients from hospital.
To seek to avoid unnecessary hospital admissions, reduce hospital length of stay and to reduce re-presentation to the Emergency Department.
To ensure appropriate and efficient transfer of care of patients into and out of the service.
To be an integral member of the multi-disciplinary team (MDT) and to establish close collaborative working relationships with key stakeholders across specialist medical and HSCP teams.
To provide physiotherapy leadership within the MDT and to the Physiotherapy Department especially in terms of clinical governance To work in an advisory capacity and as a central resource to HSCP, medical directorates and regional or national agencies.
To lead on service development, using evidence based practice, audit, lean methodology, risk assessment and implementation of change as required.
To lead on development and education of the physiotherapy team and to support education and training of basic grade and assistant staff and of Physiotherapy undergraduate and post-graduate clinical education and that of the wider MDT.
To undertake and facilitate relevant research in the clinical area, in line with hospital policies and to present and publish research outcomes.
