Key Responsibilities:
As a Senior Social Worker in Primary Care, you will play a key role in providing early intervention, assessment, and direct support to individuals and families.
Conduct psychosocial assessments to identify support needs and develop care plans.
Provide crisis intervention and safeguarding support to vulnerable individuals at risk.
Work collaboratively with a range of professionals to ensure integrated and coordinated care.
Advocate for service users, ensuring their rights, choices, and best interests are upheld.
Deliver therapeutic interventions, including counselling, emotional support, and signposting to relevant services.
Contribute to service development, helping to improve policies, procedures, and community outreach initiatives.
Ensure compliance with HSE social work policies, safeguarding frameworks, and ethical standards.
Essential Criteria:
CORU registered Social Worker (essential).
Minimum of three years post-qualification experience, ideally within a community or primary care setting.
Depth and breadth of experience in providing a social work services in community services Excellent leadership, communication, and advocacy skills.
Ability to work both autonomously and within a multidisciplinary team.
Full clean driving licence and access to transport is an advantage.
Valid Garda and overseas clearances (if applicable).
