We are looking for a Disabilities Case Manager to join our team in Enfield, Co. Meath, working closely with clients and care teams in a home-based setting.
This role is perfect for someone who thrives in leadership, care coordination, and client advocacy, ensuring high-quality, person-centred care.
What You'll Do:
Oversee and coordinate care packages, ensuring tailored and effective support.
Provide clinical supervision and leadership to community-based staff.
Develop and implement individualized care plans, updating them as needed.
Liaise with families, multidisciplinary teams, and healthcare professionals to ensure seamless care.
Carry out regular audits, compliance checks, and training to maintain high standards.
Assist with staff recruitment, onboarding, and supervision to ensure a skilled and motivated care team.
Support rostering and scheduling to ensure smooth service delivery.
What You Need:
BSc in General Nursing or Registered Intellectual Disability Nursing (or ABI experience).
2-5 years of community or general nursing experience (NMBI registration essential).
Full driver's license & private vehicle (travel required).
Strong IT skills, including Microsoft Word and Excel.
High attention to detail with a structured, proactive approach.
Excellent communication and leadership skills to coordinate care effectively.
Passion for continuous improvement in healthcare and client advocacy.
