SERVICE DESCRIPTION:
We are seeking Support Workers to join our team at Praxis Care. As a central part of our mission, you will provide quality care to individuals, fostering an encouraging environment that promotes independence.
Your role is crucial in helping individuals achieve their desired outcomes, while maintaining a service where respect and dignity are paramount.
You will work closely with Team Leaders and managers to prepare and implement individual and group support programmes.
Key Responsibilities:
* Empower individuals to develop or improve independent living skills as identified in person-centred plans.
* Work collaboratively to promote a safe working and living environment, effectively resolving situations that may arise from conflict or challenging behaviours.
* Ensure medication is stored and administered according to our Medication Policy.
* Record and log relevant information according to our reporting guidelines.
* Familiarize yourself with all administrative requirements of the service.
Requirements:
* Participate in regular appraisal and staff review processes.
* Model our company values, behaviours, and competencies at all times.
* Carry out duties in accordance with Praxis expectations, regulations, and policies.
* Be prepared to take on additional duties and responsibilities as required.
* Work flexible hours to meet the needs of the service.
Critical Skills and Competencies:
* Numeracy and literacy skills.
* Effective communication skills.
Desirable Criteria:
* Experience working within the health and social care field.
