RDRD is currently looking to recruit for the following position: Full-time project/care and case management worker.
Salary: €41,000.00 Annually

Key Responsibilities:
1. Conducts comprehensive assessments to understand the individual's needs, substance use history, and any co-occurring mental health conditions.
2. Develops personalised care plans that outline treatment goals, interventions, and timelines.
3. Coordinates with various healthcare providers, therapists, and social workers to ensure seamless care delivery.
4. Facilitates referrals to appropriate treatment programs, support groups, and community resources.
5. Collaborates with other case managers and treatment team members to ensure consistency and continuity of care.
6. Advocates for the individual's rights and needs, especially when navigating complex healthcare systems.
7. Provides emotional support and encouragement throughout the recovery process.
8. Helps individuals develop coping strategies and relapse prevention plans.
9. Monitors the individual's progress and adjusts the care plan as needed.
10. Provides ongoing support and follow-up to prevent relapse and maintain recovery.
11. Assists with discharge planning and transition to aftercare services.
12. Assists with practical needs like housing, transportation, and employment.
13. Provides education and information about addiction, recovery, and healthy lifestyle choices.
14. Facilitates family involvement and support.
15. Supports the team in setting up care plans and liaises with the general manager.
Minimum Requirements:
Applicants must demonstrate a clear understanding and knowledge of addiction; an understanding of care and case management is essential. Applicants must understand the E-Cass system and the running of reports. Applicants must have at least 3 years of service delivery experience in the field of addiction. Applicants will hold a third-level qualification in the field of addiction or the equivalent. A probationary period will apply.
