This is a fantastic opportunity to make a real impact in the community, working alongside a second Senior Physiotherapist, a Senior Occupational Therapist, and a Physiotherapy Assistant. About the Role As a Senior Physiotherapist in this Community Intervention Team, you will play a vital role in assessing, treating, and supporting patients/ older persons in the community. Your work will help individuals regain mobility, maintain independence, and improve their overall quality of life. Key Responsibilities: Provide comprehensive physiotherapy assessments and treatments for patients with a variety of conditions, including musculoskeletal, neurological, and age-related conditions. Work collaboratively with the Senior OT, second Senior Physiotherapist, and Physiotherapy Assistant to deliver patient-centred care. Develop and implement individualised treatment plans tailored to each patients specific needs and goals. Support early discharge from hospitals by providing rehabilitation and reablement interventions in the community. Liaise with GPs, nurses, and other healthcare professionals to ensure coordinated and seamless patient care. Engage in regular supervision, peer learning, and professional development opportunities to enhance your expertise. Minimum of three years of post-graduate Physiotherapy experience, with at least one year at Senior level (preferably in a community or rehabilitation setting). Strong experience in, Rehabilitation, falls prevention, Musculoskeletal, and/or Neurological Physiotherapy. Ability to work independently and as part of a team, demonstrating leadership and initiative. Excellent communication, interpersonal, and problem-solving skills. Full clean drivers license and access to a car for community visits.
