CHO Dublin North City and County is one of nine Community Healthcare Organisations across the country and is responsible for providing care and services to our geographical population of approximately 679,579. Mellany McLoone is Chief Officer for CHO DNCC and has full responsibility and accountability for the delivery of all Community Healthcare services in the area. Community Healthcare Services are the broad range of services that are delivered outside of the acute hospital setting.
Description
The Community Intervention Team Integrated Health Areas of Dublin North County and Dublin North City & West is an acute HSE nursing service, established in 2006, which has developed and expanded over the years. The main purpose of the CIT is to provide acute community nursing care enabling patients to be cared for in the community as much as possible. The aims of the CIT service are as follows:
The aim of the CIT service is to:
* Provide safe, high-quality care to clients living with chronic disease in the community setting, including clients who are actively receiving chemotherapeutic treatment for cancer.
* Maximise the ability of people with chronic illness to receive care in the community setting.
* Assist in facilitating early discharge of patients from acute care.
* Prevent avoidable hospital admissions by supporting and collaborating with community-based health professionals and the PHN service.
Services delivered in the CIT clinic include:
* Central line care, including ambulatory chemotherapy pump disconnection.
* Administration of Systemic Anti-Cancer Therapy (SACT) including oral therapy, bisphosphonates, and Herceptin subcutaneous.
* Administration of non-SACT medications.
The clinic operates 5 days a week, Monday to Friday, from 08:00 - 18:00. Key to the success of the service are the working relationships with both hospital-based and community-based health professionals, all working together to support the delivery of high-quality care to patients.
The post holder has responsibility for the provision of professional nursing care to meet the individual needs of the patient population and is accountable for the delivery of such care. He/she will be expected to maintain a high standard of nursing care and to liaise closely with and support community nursing.
