HSE Mid West is hiring a Social Worker, Senior Medical, Inclusion Health.

HSE Mid West is developing an Inclusion Health service, primarily based at University Hospital Limerick and providing outreach services across the Mid-West.
The role of the Senior Medical Social Worker in Paediatric Inclusion Health will be to lead the design, implementation, and delivery of the social work component of the Inclusion Health service, and to work in an integrated fashion with the other elements of the service.
This will involve collaboration with hospital and community stakeholders to identify the areas of highest clinical need, evaluate the barriers to health and healthcare for the relevant populations, and develop and deliver care pathways to overcome them.
