About the Role
The Programme Facilitator plays a key role in supporting individuals with complex needs to achieve and maintain valued roles in their community through learning, social and recreational opportunities.
Dunboyne Day Centre is a Physical and Sensory Day centre which supports people in the Dunboyne and surrounding area.
The post-holder will facilitate individuals in their programmes, passionately supporting their educational, developmental, recreational, social and occupational activities.
The role requires the investigation of new opportunities and the requirement to advocate in the best interest of the people in the care, service and support of the Rehab Group.
What We're Looking For
* Minimum QQI Level 5 in Social Care or equivalent
* A third level graduate, Health & Social Care or equivalent is desirable.
* Experience supporting adults with Varying disabilities.
* Programme facilitation skills an advantage.
* 1-year minimum experience of working in a relevant Setting with the experience of facilitating and implementing person-centered plans, experience in the design, development and provision of programmes and activities to meet the needs, choices and abilities of service users including providing support in the area of independent living skills and community integration for persons with physical and sensory disability
Your Responsibilities
1. Act as Key Worker for Service Users, identifying goals and supports required and maintaining records in order to facilitate the Person-Centred Planning process.
2. Develop and deliver Person-centred programmes and maintain records of same in order to meet the needs of Service Users that have been identified through the Person-Centred Planning process.
3. Support individuals and groups in accessing their community by facilitating activities, programmes and any other reasonable duties as deemed appropriate by the Programmes Supervisor or Community Services Manager in order to meet identified needs.
4. Acting on initiative as a lone-worker, maintain records and liaise with relevant team members and communicating with external agencies & family members to ensure that Service Users receive the highest standards of service.
