Job Description
You'll work closely with a multidisciplinary team to provide support and guidance, empowering individuals to overcome challenges and achieve independent living.
Key Responsibilities:
* Act as a key worker for individual young people, providing personalized support and guidance.
* Develop and maintain individual placement plans in line with statutory care plans, ensuring comprehensive and effective support.
* Collaborate with external agencies, including housing, education, and social support services, to ensure seamless coordination and delivery of support.
* Maintain accurate and up-to-date records, adhering to health and safety standards and regulatory requirements.
* Participate in a 7-day, 24-hour roster, demonstrating flexibility and commitment to the role.
Requirements:
* Degree in Social Care or a related field, with a strong foundation in principles and practices.
* Experience in residential care or working with children with complex needs, with a proven track record of success.
* Knowledge of HIQA standards, Child Protection, and Therapeutic Crisis Intervention, with the ability to apply this knowledge in practice.
* Excellent communication skills, with the ability to work effectively with individuals, teams, and external partners.
* Strong teamwork mindset, with a focus on collaboration and mutual support.
* Full, clean driver's license, with a valid qualification.
