Social Work Team Leader - Home Sharing Services Full time – permanent 35 hour working week Are you a dynamic and experienced Social Worker who is passionate about person-centred supports and inclusive community living? We are seeking a Social Work Team Leader to lead and develop our Home Sharing Services within Horizons.
About the Role:
As the Social Work Team Leader in Home Sharing, you will oversee the delivery of high-quality, safe and person-centred Home Sharing supports for children and adults in line with the HSE National Guidelines for Home Sharing (2016).
You will provide leadership, supervision and guidance to a team of Social Workers, ensuring compliance with national policy, organisational policies and professional standards as set by CORU (Health and Social Care Professionals Council).
You will work closely with Host Home Sharing Families (HSF), the people we support and multidisciplinary teams in Children's and Adult services to promote inclusive and meaningful home sharing placements in the community.
Rooted in a rights-based approach, you will guide your team and HSF's in embracing positive risk-taking – supporting individuals to make choices, explore opportunities and live fully, while managing risk in a balanced and ethical way.
Reporting to: Operational Line Management: Regional Manager Clinical Supervision: Principal Social Worker Key Responsibilities:
Lead and manage the Home Sharing Social Work team, ensuring effective service delivery and compliance with regulatory and quality standards. Promote and implement the HSE National Home Sharing Guidelines (2016) across all placements and practice. Support the recruitment, assessment, training and ongoing development of Host Home Sharing Families. Oversee the matching of people we support, whether children or adults, with suitable Home Sharing Families. Foster collaborative working relationships with families, the people we support, community stakeholders and the HSE. Drive innovation, service expansion and person-centred planning within the Home Sharing model. Essential Requirements:
A recognised qualification in Social Work and current CORU registration. Minimum of 3 years post qualification experience including 2 years in disability services, fostering or related fields. Proven experience in team leadership or management within a social care setting. A strong understanding of positive risk-taking in the context of person-centred, rights-based supports. In-depth knowledge of the HSE National Home Sharing Document (2016) and a commitment to its implementation. Strong knowledge of the Assisted Decision-Making (Capacity) Act 2015. Strong knowledge of the HSE-Tusla Joint Protocol for Interagency Collaboration. Excellent communication, supervision, administration and organisational skills. Experience in managing budgets and family-based support payments. A full clean driving license and access to own vehicle.
