Job overview
To be a key member of the Diabetes team structure providing specialist clinical management to patients with diabetes using specialist skills and knowledge to facilitate the effective management of patients with diabetes. The outcome of this activity is to avoid unnecessary hospital admissions via more effective management and prevention of acute exacerbations and the offer of early supported discharge to patients admitted to secondary care.
Main duties of the job
* To work as an accountable and autonomous practitioner, by leading in provision of diabetes care for patients, within Lincolnshire. This is to include clinical care, education and training for patients and staff within LCHS.
* To provide advanced clinical care using high level assessment to make a diagnosis, initiate medical/nursing treatments, refer appropriately and co-ordinate the patients journey across all parts of the health, social care and voluntary sector systems whether in own home or care settings.
* Improve clinical outcomes for these diabetes patients by enabling them to function independently by increasing their choice to remain in their own home/community and reduce the need for, or prevent inappropriate hospital admission.
* To be responsible for a delegated budget to support specialist function within the Diabetes service.
* To be responsible for performance and target levels as identified by LCHS and the diversions.
* Identification of very high intensity users with complex needs using risk stratification systems and other sources of information.
* To support the Diabetes team to meet agreed quality and performance targets.
* Take a Leadership role in the continuous development of the service.

Detailed job description and main responsibilities
* Undertake comprehensive clinical assessment of the complex health and psycho-social care needs of patients with diabetes.
* Establish individual’s functional capabilities with regard to diabetes clinical management.
* Plan, implement, monitor and review therapeutic interventions with individuals who have diabetes and their carers.
* Investigate and diagnose the unwell individual with diabetes.
* Lead complex medicine management review and prescribe medicine and appliances for patients using independent prescribing skills.
* Lead on the development or review of clinical policies and guidelines related to the care of patients with diabetes within the business unit.
* Take a Leadership role in the continuous development of the service.
* Line management of Band 6 diabetes specialist nurses in the area.
Person specification
* Non-medical Prescriber or willing to work towards.
* Masters level qualification or equivalent experience and knowledge.
* In depth understanding of the management of diabetes.
* Advanced clinical assessment skills.
* Ability to prioritise and manage workload in such a way that takes account of patient’s urgent needs.
* Flexible approach to meet service need, self-motivated and enthusiasm for the role.
* Management or leadership qualification.
* Experience in community/primary care.
