In accordance with established mission, policies, and procedures of Friendship at Home, including Resident Rights, and/or specific instructions from the Care Coordinator Manager, the Senior Care Coordinator provides an individualized plan of care for each member using effective and efficient case management techniques.

Responsibilities:
* Provide 5-Star service to our lovely residents!
* Schedule and complete admission assessments of new members which may include a mini-mental exam, home assessment and level of care guidelines (ADLs/IADLs) on an annual and as needed basis.
* Complete quarterly member check ins with members not currently receiving care.
* Schedule and complete quarterly or PRN ADL assessments to evaluate appropriate level of care is in place.
* Manage a caseload of members, including quarterly communication with members and/or their families, assessments of needs, coordinating and scheduling services (both provided by the membership plan and those not provided by the membership plan).
* Initiate the member's care plan and maintain detailed records of all member activity in the member’s medical record.
* Initiate care coordination for members and supervise the provision of care rendered by the credentialed vendor.
* Independently lead and direct the care needed and recommend and coordinate services for members on the assigned caseload.
* Represent Friendship at Home at professional meetings, networking opportunities, community organization meetings, etc upon request.
* Assist with the development and updating of an outside referral directory for members.
* Continually update clinical assessment skills and knowledge of available resources for the senior population.
QUALIFICATIONS:

* LPN Nursing License, BSW or a Bachelor’s Degree preferred in a related field or an Associate’s Degree with a minimum of 3 years of experience working with seniors in a health care environment.
* Experience in case management or related field with the senior population including demonstrated skills in assessment, planning, evaluating plans of care, organizing and maintaining detailed filing systems; familiarity with updated clinical assessment methods and community resources; an understanding of reimbursement criteria for Medicare and Medicaid.
* Excellent interpersonal skills with demonstrated ability to interact and communicate effectively with members, families, staff, providers, professional community and others.
PHYSICAL REQUIREMENTS/WORKING CONDITIONS:
Normally works in a well-lighted, comfortable office setting. Must be able to work efficiently with frequent interruptions. Must be able to move freely throughout the facility and able to lift (unassisted) 50 lbs. Must be able to drive to and from members’ homes in Central Ohio. Physical effort is required to walk within the office and to and from the members’ homes, appointments and community.
