Home Instead, Ireland's largest private home care provider, seeks a skilled CARE Manager to lead our North Dublin County operations.
Key Responsibilities:
* Conduct consultations and prepare comprehensive care plans for clients.
* Conduct thorough risk assessments in clients' homes and implement mitigation strategies as needed.
* Monitor, mediate, and manage client and CAREGiver concerns and issues efficiently.
* Perform Quality Assurance (QA) visits in client homes, develop and review care plans, and conduct service enquiries and care consultations as required.
* Evaluate client care plans against their specific needs, collaborating with family members and healthcare professionals when necessary.
* Ensure delivered care meets HCCI, ISO & Franchise Standards, local policies, and Health Service Executive (HSE) requirements.
* Provide ongoing CAREGiver Supervisions and maintain up-to-date records.
* Mentor and support a large team of CAREGivers to excel in their roles.
* Develop and maintain strong relationships with clients, families, and caregivers.
* Establish and enhance connections with healthcare professionals and multi-disciplinary teams in hospitals and community settings.
* Report and consult with senior management on clinical matters as appropriate.
* Participate in clinical audits identified by the organization.
* Assist with delegated tasks from the Management Team.
Requirements:
* QQI Level 5 Social, Healthcare degree preferred.
* At least 3 years of experience in home care, healthcare, or a senior-related industry.
* Equivalent combination of education and work experience may be considered.
* Strong computer literacy skills are essential.
