Job Title: Case Manager / Social Care Leader
Location: Wicklow
Department: Disability Services Team
Reports to: Disability Manager
Roles and Responsibilities:
* Collaborate as part of the Disability Service Team to enhance service provision for individuals with disabilities.
* Build relationships with relevant stakeholders to ensure maximum service delivery.
* Identify service gaps and contribute to the development and prioritization of service improvements.
* Work closely with clients, families, and multidisciplinary teams to develop and review service options.
* Prepare and present costed business cases for the provision of individual client services as required.
* Lead, coordinate, and participate in multidisciplinary case management reviews.
* Foster interagency and intersectoral collaboration with statutory, voluntary, and private service providers.
* Act as the Assessment / Liaison Officer under the Disability Act (2005) as needed.
* Maintain accurate records and provide data and reports in compliance with professional service standards.
* Manage budgets, expenditure, and resources efficiently, preparing financial management reports as required.
* Oversee procurement processes and ensure adherence to relevant policies.
* Provide reports on complaints and contribute to research and development projects.
Qualifications and Experience:
* Significant experience in Disability Services, with expertise relevant to case management.
* Experience in case management processes such as placement, discharge liaison, and care coordination.
* Proven experience working in multidisciplinary teams within Disability Services.
Skills, Competencies, and Knowledge:
* Professional Knowledge & Experience
o In-depth understanding of the Disability Act 2005 and Assisted Decision-Making (ADM) Act 2015.
o Knowledge of the Health Act 2004 & 2007, HIQA regulations, and legislation relevant to service procurement.
o Extensive experience with HSE and voluntary sector services.
* Evaluating Information, Problem Solving & Decision Making
o Strong analytical, problem-solving, and decision-making abilities.
o Ability to understand and communicate complex issues, with a sound rationale for decision-making.
o Confident in resolving issues and making decisions while knowing when to escalate matters.

o Excellent communication and interpersonal skills, with the ability to engage with diverse stakeholders.
o Skilled at presenting complex information clearly and writing comprehensive reports.
* Planning, Organizing & Delivery of Results
o Strong planning and organizational skills, with the ability to manage and prioritize multiple caseloads.
o Proven ability to manage time efficiently and develop solutions to improve service delivery.
* Building & Maintaining Relationships (Teamwork & Leadership)
o Strong ability to foster collaborative working relationships and lead by example.
o Effective leadership skills with experience in coaching, support, and teamwork.
