Description
Location of Post: The specific location of the post will be confirmed at job offer stage.
A panel may be created from which current permanent & specified purpose vacancies of full time or part time duration may be filled.
Purpose of the Post: The post holder will manage the delivery of the Home Support Service to clients within a defined geographical area, which is generally based on a number of Primary Care Teams.
The person appointed to this post will work within the Community Healthcare Area; working as part of multi-disciplinary teams delivering a coordinated approach to client care.
The Home Support Department provides a responsive and standardised application, assessment, approval, delivery and review process for the following range of services:
* Healthcare Support Assistants to all care groups
The Service’s funding is allocated for older people, with exceptional approval for clients aged under 65 years where all other avenues for assistance have been exhausted.
Objectives of the Service:
* Facilitate timely discharge of older people from acute hospitals, including acute psychiatric units, and community hospitals.
* Reduce inappropriate admission of older people to acute care or residential care.
* Reduce pressures on Emergency Department.
* Support older people to continue to live, or return to live, in their own community.
* Support carers so that they might be able to continue to provide care for older people.
* Provide services within the annual resource allocation.
Reporting Relationship: The post holder will report directly to the Service Manager for Older Persons or her designate. A team of Homecare Support Assistants and a Clerical Officer will report to the successful candidate.
Eligibility Criteria Qualifications and/or Experience: Eligible applicants will be those who on the closing date for the competition:
(a) Demonstrate relevant management ability and experience within a health or social services setting.
And
(b) Possess the requisite knowledge, ability and experience to fulfil the duties of the post.
Health: Candidates for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.
Character: Each candidate for and any person holding the office must be of good character.
