Social Worker
My client, a provider of Mental Health services in Westmeath, is seeking a dedicated and compassionate Social Worker with at least three years of experience to join the Psychiatry of Later Life Services team. The successful candidate will play a key role in providing psychosocial support to older adults experiencing mental health challenges, ensuring holistic, person-centred care within a multidisciplinary framework.
Key Responsibilities of Social Worker
* Conduct comprehensive psychosocial assessments and develop intervention plans tailored to individual patient needs.
* Provide therapeutic support, crisis intervention, and counselling to patients and their families.
* Advocate for patients’ rights and facilitate access to appropriate community services and supports.
* Collaborate closely with multidisciplinary teams, including psychiatrists, psychologists, nurses, occupational therapists, and other allied health professionals.
* Contribute to the planning and implementation of care plans for older adults with complex mental health needs.
* Assist in discharge planning and liaise with community-based services to ensure continuity of care.
* Maintain accurate and up-to-date documentation in line with professional and organisational standards.
* Provide education and training to families and caregivers on mental health conditions, coping strategies, and available support networks.
* Participate in team meetings, case conferences, and quality improvement initiatives within the service.
Requirements of Social Worker
* A recognised qualification in Social Work and registration with CORU.
* A minimum of two years’ post-qualification experience as a Social Worker, with experience in mental health or older adult services preferred.
* Knowledge of mental health issues affecting older adults and an understanding of relevant legislation, policies, and best practices.
* Experience in risk assessment, crisis intervention, and safeguarding vulnerable adults.
* Ability to work both independently and as part of a team in a fast-paced healthcare environment.
* Commitment to ongoing professional development and adherence to ethical standards in social work practice.
