Community Support Facilitator - (Bespoke Evening Services)
Are you driven by a passion for empowering individuals with intellectual disabilities and ASD to live fulfilling lives within their community? Do you have a natural ability to connect with others and facilitate meaningful social interactions? If so, we have an exciting opportunity for you to become a Community Support Facilitator as part of our Bespoke Evening Services. (This is initially a 1 year fixed term contract. Contracts are with the Service. (Pay grade HSE Care Assistant ID)

Key Responsibilities:
Community Engagement:
* Act as a facilitator for community integration, supporting individuals to participate in social, recreational, and vocational activities tailored to their interests and preferences.
* Foster connections and relationships within the community, promoting inclusivity and acceptance of individuals with disabilities.
Personalised Support:
* Provide individualised support to enhance independence and well-being, assisting with activities of daily living, skill-building, and goal attainment.
* Implement behaviour support strategies and interventions to promote positive behaviour and minimise challenges, in collaboration with the support team.
Empowerment and Advocacy:
* Empower individuals to advocate for themselves and make informed choices about their lives, promoting self-determination and autonomy.
* Advocate for the rights and needs of individuals within the community, ensuring their voices are heard and respected.
Documentation and Collaboration:
* Maintain accurate records of support provided, including daily notes, goals, and outcomes, in compliance with best practice & organisational standards.
* Collaborate with the Social Care Leader and interdisciplinary team members to develop and implement individualised support plans, incorporating feedback and adjustments as required.

Situated in the heart of Gorey Town, our Bespoke Evening Support Service offers convenient access to a wealth of local amenities and cultural attractions.
POSITION 1: Three evenings a fortnight i.e. 24 hours a fortnight, excluding Mondays and weekends, flexibility is required to ensure ample opportunity for individuals to engage in personalised activities and pursuits that align with their interests and preferences.

Qualifications/Requirements:
* A minimum of QQI Level 5 in Health Care Support/Social Care (A Diploma or Degree in Social Care or other relevant discipline advantageous).
* A minimum of one year's experience of working with people with an Intellectual Disability/ASD required.
* Must hold a full clean driving license and willing to drive company vehicles.
