Role: Clinical Care Manager
Job description
We are looking for a dedicated Clinical Care Manager to lead and deliver person-centred care, ensuring that each service user’s preferences, values, and needs are at the heart of everything we do, while adhering to home support best practices.
Key Responsibilities:
1. Overseeing the development of Person-Centred Care Plans: Managing and collaborating with a team of Supervisors to ensure that service users and their support networks lead the development of tailored person-centred care plans that reflect each individual’s unique preferences, goals, and needs.
2. Promoting Service User Autonomy: Foster an environment that encourages service users to make informed decisions about their care and support them in maintaining independence.
3. Staff Supervision & Training: Mentor and train staff in the principles of person-centred care, ensuring that every team member is equipped to treat service users as active partners in their care journey.
4. Ensuring Quality & Safety: Monitor care outcomes, ensure high standards of care delivery, and lead quality improvement initiatives that keep the service user’s experience at the forefront.
5. Collaboration: Work closely with a multidisciplinary team to coordinate and deliver seamless care that meets the holistic needs of each service user.
6. Advocacy: Act as a strong advocate for both staff and service users, ensuring that their voices are heard, and their rights are respected in every aspect of the provision of home support services.
Qualifications & Experience (Candidates must):
1. Licensure: Registered on the general division of the Nursing and Midwifery Board of Ireland live register.
2. Experience: At least 5 years post registration experience with a minimum of 2 years in a community-based role.
3. Qualification: Management and home support desirable. Must be willing to undergo further training as required.
4. Other: Full current driving licence and access to own car.
5. IT proficiency, including the use of care management systems and electronic records.

Job Types: Full-time, Graduate
Pay: €50,000.00-€55,000.00 per year
Schedule:
1. Monday to Friday: 9:00 AM to 5:00 PM, primarily office-based with some flexibility.
Note: It is a temporary role with the possibility to become permanent.
