Social Worker We are currently seeking a dedicated Social Worker to join a dynamic multidisciplinary team, providing essential advocacy and support services to individuals and their families.
This role involves managing a caseload of varying complexity while ensuring that each person receives the highest level of care and access to the services they need.
Key Responsibilities:
Manage a diverse caseload, addressing both individual and family needs.
Advocate for the rights and entitlements of individuals, ensuring they are informed and supported.
Facilitate advocacy sessions, guiding individuals through self-advocacy and group advocacy initiatives.
Identify and address unmet needs, working closely with the team to develop appropriate solutions.
Participate in care planning and multidisciplinary meetings, collaborating with staff and clinicians to enhance support services.
Provide safeguarding support, assisting designated officers in protecting vulnerable individuals.
Contribute to funding applications and reports, ensuring accurate documentation and compliance with best practices.
Maintain professional standards in line with current legislation and organizational policies.
Requirements:
Recognized Social Work qualification (NQSW/CQSW) or equivalent.
CORU registration (or in the process of registering).
Minimum one year of professional experience in social work.
Experience working within a multidisciplinary team.
Strong IT skills for documentation and reporting.
Full, clean driving license and access to a car.
