Midlands Simon Community is recruiting a Project Worker.
Job Purpose
The Project Worker will support the needs of Midlands Simon Community Service Users by responding appropriately and providing interventions that facilitate their settlement into the community and by assisting them to maintain their independent living to maximise their potential and move out of homelessness.
Key Responsibilities
1. Engage with and build a professional trusting relationship with Service Users, treating them with dignity and in line with the values of Midlands Simon Community and the organisation’s equal opportunities policy.
2. Support Service Users participation in Midland Simon's Service Users Programme.
3. Promote Midlands Simon Community’s mission, vision, values and strategic objectives.
4. Work from Trauma Informed Care model and/or an appropriate evidence-based care model.
5. Encourage independence and self-determination in Service Users.
6. Responding to referrals made by the Local Authority which need an emergency response:
7. Carry out an assessment when indicated by the Local Authority that a referral needs an emergency response.
8. Following an assessment of emergency cases, select and implement a range of interventions from the suite of interventions available to the service.
9. Provide a report of actions in response to emergency referrals to the Head of Services and to the Local Authority.
10. Referral, Assessment and Development of a Personal Action Care Plan.
11. Plan and organise a full induction for all new Service Users.
12. Carry out an assessment with potential Service Users in line with Midlands Simon Community’s equal opportunities policy.
13. Identify and explore housing options with Service Users.
14. Work with Service Users in assessing their needs and achieving their goals.
15. Work with Service Users to develop a holistic needs-based personal action care plan.
16. Implementing the Personal Action Care Plan.
17. Support Service Users to move into their new homes.
18. Demonstrate a proactive response to working with the Service User to prevent and respond to issues that may put the Service User’s tenancy at risk.
19. Key work with Service Users to ensure that they have the basic skills required for living independently and maintaining their new homes.
20. Liaise effectively with a range of service providers, both voluntary and statutory, to ensure services are available to the Service Users.
21. Accompany Service Users to statutory and other agencies and to act as an advocate as necessary.
22. Support Service Users to claim all relevant benefits and take up all opportunities for health improvement, training and personal development.
23. Identify, in partnership with Service Users, any risks to Service Users in losing their tenancy, and in consultation with the head of services, develop appropriate responses.
24. Ensure that the services are of high quality in line with the approved Housing Bodies Regulator Authority National Quality Standards for homeless services, based on best practice and evidenced based models and to the responsive needs of Service Users.
