Job Description:
CNM 11 Registered Children’s Nurse required for the Paediatric Diabetes Service Midland regional Hospital Mullingar.
To cover maternity leave. This is a very specialised service and would require the candidate to be a Registered Children’s Nurse with experience in caring for children with Diabetes or preferably have a qualification in this area.
From 31st March as follows:
Shift times:
08:30am to 1700pm (Monday, Wednesday and Thursday)
08:30am to 1730hrs (Tuesday)
08:30am to 13:30pm (Friday)
The candidate would be working as part of a MDT in the Paediatric diabetes service which consists of Paediatric consultant, ANP and CNM2.
DAILY TASK AND EXPECTATION:
Monday:
1. Equipment calibration (QC meter calibration once a week and DCA calibration once a month), inventory checks and placing orders for adequate supplies for diabetes care.
2. Planning, organising and coordinating virtual and in-person reviews of children with Type 1 and Type 2 diabetes with their families.
3. Assessing and planning treatment plans for the cohort after discussing with the MDT team.
4. Respond to the emails and phone calls.
Tuesday:
Clinic Day (Preparation and Review)
Wednesday:
1. Planning, organising and coordinating virtual and in-person reviews of children with Type 1 and Type 2 diabetes (with their outstanding clinical issues).
2. Coordinate with Paediatric Diabetes secretary regarding dates for follow-up nurse-led appointments from last clinic.
3. Next week Clinic prep.
4. Respond to the emails and phone calls.
Thursday:
1. Pre and Post Clinic MDT meetings.
2. Planning, organising and coordinating virtual and in-person reviews of children with Type 1 and Type 2 diabetes (with their families).
3. Respond to the emails and phone calls.
Friday:
1. Coordinate with the paediatric diabetes secretary in organising the recall text and calls for next week clinic, nurse-led and virtual reviews.
2. Coordinate with the paediatric ward and organise adequate diabetes management supplies for the weekend in case of an admission.
3. Planning, organising and coordinating virtual and in-person reviews of children with Type 1 and Type 2 diabetes (with their families).
Daily Tasks (Subject to change):
If there is a child with Type 1 Diabetes admitted in the ward (Newly Diagnosed Type 1 Diabetes, stabilisation etc.), re-organise tasks based on the priorities. The focus will shift to: Providing structured education, support to the child and their family in the management of diabetes and coordinate with the medical team in the stabilisation and discharge of the child followed by organising frequent follow-ups and continued education and support at community level.
Professional / Clinical Responsibilities:
* Manage patient care to ensure the highest professional standards using an evidence-based, care planning approach.
* Provide a high level of professional and clinical leadership.
* Be responsible for the co-ordination, assessment, planning, delivery and review of service user care by all staff in designated area(s).
* Provide safe, comprehensive nursing care to service users according to the Code of Professional Conduct as laid down by the Bord Altranais agus Cnáimhseachais na hÉireann (Nursing Midwifery Board Ireland) and Professional Clinical Guidelines.
* Practice nursing according to: Professional Clinical Guidelines, National and Area Health Service Executive (HSE) guidelines, Local policies, protocols and guidelines, Current legislation.
* Manage own caseload in accordance with the needs of the post.
* Participate in teams / meetings / committees as appropriate, communicating and working in co-operation with other team members.
* Facilitate co-ordination, co-operation and liaison across healthcare teams and programmes.
* Collaborate with service users, family, carers and other staff in treatment / care planning and in the provision of support and advice.
* Communicate verbally and/or in writing results of assessments, treatment / care programmes and recommendations to the team and relevant others in accordance with service policy.
* Plan discharge or transition of the service user between services as appropriate.
* Ensure that service users and others are treated with dignity and respect.
* Maintain nursing records in accordance with local service and professional standards.
* Adhere to and contribute to the development and maintenance of nursing standards, protocols and guidelines consistent with the highest standards of patient care.
* Evaluate and manage the implementation of best practice policy and procedures admission and discharge procedures, control and usage of stocks and equipment, grievance and disciplinary procedures.
* Maintain professional standards in relation to confidentiality, ethics and legislation.
* In consultation with the DNM and other disciplines, implement and assess quality management programmes.
* Participate in clinical audit as required.
* Initiate and participate in research studies as appropriate.
* Devise and implement Health Promotion Programmes for service users as relevant to the post.
* Operate within the scope of practice - seek advice and assistance from his/her manager with any cases or issues that prove to be beyond the scope of his/her professional competence in line with principles of best practice and clinical governance.
