Minimum Education & Skills required Third-level graduate, Health & Social care or equivalent is desirable Attendance at training as deemed appropriate by the management of the service.
Minimum 2 years previous experience of working with people with a disability.
Essential - A full, clean driving license, with a minimum of 2 years' experience is required Job Duties & Responsibilities To collate, assessment and intervention information and expertise for the purpose of developing an individualized community support service.
To document information, update records, and complete necessary paperwork for communication and accountability purposes.
To ensure all interaction with clients is approached with dignity, respect, equality and incorporates choice.
