Our client, a reputable organization providing quality residential care and support for young people aged 11-17, is seeking a dedicated Social Care Worker to join their team. The ideal candidate will be responsible for delivering exceptional care and guidance to help young people navigate their journey to independence and adulthood.
Key Responsibilities:
* Provide short to medium term transitional care for young people moving to and from Special Care Services
* Develop and implement personalized support plans to promote independence and well-being
* Collaborate with multidisciplinary teams to ensure seamless care delivery
Requirements:
* Level 7/8 in Applied Social Care
* Flexibility to work a residential roster
* Experience in Mainstream services desirable
* Car and Full Driving Licence
