Job Title: Programme Facilitator

About the Role:
* Promote and support independent living and community integration.
* Facilitate educational, developmental, recreational, and occupational programmes and activities as appropriate.
Key Responsibilities:
* Develop and deliver person-centred programmes to meet the needs of service users.
* Maintain required records and be a key worker to service users.
* SUPPORT INDIVIDUALS AND GROUPS IN ACCESSING THEIR COMMUNITY TO MEET IDENTIFIED NEEDS.
* Observe all rules and regulations of the RehabCare services.
Requirements:
* Minimum QQI Level 5 in Social Care or equivalent.
* Experience supporting adults with intellectual disability and people enduring mental health issues.
* Experience of working in a Day Service Setting (1 Year min).
* Facilitating and implementing a person-centred plan.

Estimated Salary: €45,000 - €60,000 per year
