Job Title Home Care Co Ordinator Reporting To Line Manager Summary of Role The Home Care Coordinator is responsible for coordinating and managing home care services, ensuring smooth operations, and collaborating with staff and clients to deliver quality care The Homecare Coordinator will manage a team of System Administrators/Scheduling Assistants to coordinate the day to day activities related to scheduling care visits ensuring the most effective and efficient visiting runs are developed to ensure maximisation of individual staffs capacity.
To prepare, maintain and constantly review all visiting schedule.
To perform clerical duties related to the scheduling functions to ensure that Drumcondra Home Help Care Services goals are consistently met.
Duties and Responsiblities Oversee the delivery of home care services, ensuring they are provided effectively and efficiently.
Manage administrative tasks related to client care, including scheduling, communication, and record-keeping.
Monitor and evaluate the quality of care provided to clients.
Address client and family concerns promptly and professionally.
Staff Management: Supervise and support System Administrators/Scheduling Address performance issues and implement corrective actions as needed.
Client Communication and Collaboration: Communicate regularly with Care Manger to understand needs and preferences of clients and their families.
Collaborate with other healthcare professionals within the HSE.
Attend meetings as and when required by line manager Operational Support: Maintain accurate and up-to-date client records.
Prepare reports and documentation as required.
Other Duties: May be required to assist with other duties as assigned by management.
