Job Role
This role is based at our Home Instead Meath office, covering clients across Meath County. The successful candidate will be responsible for managing the care of our clients, ensuring their needs are met and exceeded.
Key Responsibilities:
* Conduct consultations and prepare comprehensive care plans tailored to each client's needs.
* Perform risk assessments in clients' homes, identifying and mitigating any potential risks.
* Maintain open communication with both clients and CAREGivers, addressing concerns and resolving issues promptly.
* Carry out regular Quality Assurance (QA) visits to ensure care plans align with clients' needs, consulting with family members and healthcare professionals as necessary.
* Verify that delivered care meets HCCI, ISO & Franchise Standards, local policies, and Health Service Executive (HSE) requirements.
* Mentor and support a team of CAREGivers, promoting effective performance and adherence to company standards.
* Foster strong relationships with clients, families, and healthcare professionals, building networks and enhancing existing connections within hospitals and community settings.
* Participate in clinical audits, providing expert input and contributing to the organization's continuous improvement.
* Undertake additional duties as assigned by senior management, working collaboratively to drive business success.
Requirements:
* QQI Level 5 Social Healthcare degree or equivalent qualification preferred.
* A minimum of 3 years' experience in home care, healthcare, or a related field; an equivalent combination of education and work experience may be considered.
