Key Responsibilities:
* Work independently with a child and their family to deliver high-quality, personalized care
* Develop and maintain relationships with families and multidisciplinary teams
* Monitor and report any changes or concerns regarding the child's health
Requirements:
* Previous experience working one-on-one with children in their own homes
* Compassionate and reliable individual capable of working on their own initiative
