This assessment is necessary for insurance providers to help each member individually with their health and well-being.
Responsibilities: Conduct Health Risk Assessments (HRA) on insured plan patients in their homes or plan designated areasIdentify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visitObtain vital signs, measure BMI, review pharmacological therapy, and conduct a physical examinationConduct necessary screenings to capture HEDIS measures and close gaps in careEducate and counsel patients and families on any conditions identified during assessmentTravel reimbursement for hotel stay (when applicable)Equipment, training, and supplies providedLiability coverage providedRequirements: Active Nurse Practitioner (NP) state license and board certificationsMust have at least 1 year experience preferredMust have valid state driver license and automobile insuranceCandidate will be required to travel within the county or counties selectedStrong computer skills: including but not limited to Word, Excel, EMRs, and web-based applications
