Job Description:
The risk assessment provider is responsible for the documentation and assessments of high-risk conditions for patients in their homes. This assessment is necessary for insurance providers to help each member individually with their health and well-being.
Responsibilities:
* Conduct Health Risk Assessments (HRA) on insured plan patients in their homes or plan designated areas
* Identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit
* Obtain vital signs, measure BMI, review pharmacological therapy, and conduct a physical examination
* Conduct necessary screenings to capture HEDIS measures and close gaps in care
* Educate and counsel patients and families on any conditions identified during assessment
* Travel reimbursement for hotel stay (when applicable)
* Equipment, training, and supplies provided
* Liability coverage provided
Requirements:
* Active Nurse Practitioner (NP) state license and board certifications
* Must have at least 1 year experience preferred
* Must have valid state driver license and automobile insurance
* Candidate will be required to travel within the county or counties selected
* Strong computer skills: including but not limited to Word, Excel, EMRs, and web-based applications
