About the Role
Cpl Healthcare, in partnership with a non-profit homeless service provider, is expanding their team in Clare.
This role offers a competitive salary range of €36,687 - €39,856.49 and a full-time, 1 year fixed term contract.
The successful candidate will work as part of the housing team to deliver a 'Housing First' approach to individuals experiencing long term homelessness in County Clare.
Key Responsibilities
* Provide a safe and welcoming environment for clients who are out of home or at risk, including the provision of basic physical care and support within the service, respecting consumer choice and self-determination.
* Establish trusting relationships with clients who are living on the streets or in temporary accommodation and engage them in work that enables them to move to a more stable lifestyle.
* Undertake assessments and support planning with clients as the lead agent using the Care and Case Management system and protocols.
* Develop methods and structures to support clients with their individual housing, welfare rights and entitlements, and access move on accommodation.
* Assist tenants to create and sustain a home, implementing practices in line with tenancy settlement indicators.
* Build up and maintain a portfolio of accommodation providers in social housing, homeless sector and private rented accommodation.
* Accompany and provide support to clients on appointments to other services such as doctors, counsellors, and court.
Requirements
* Degree in social care, youth work or related discipline.
* Knowledge and understanding of homelessness and issues that affect people who are or have been homeless.
* Ability to plan and implement programmes.
* Report writing abilities and IT skills.
* Flexibility and communication skills.
* Full driver's license with access to car (desirable).
Desirable Experience
* Working with customers who are or have been homeless.
* Working with customers identified as having a dual diagnosis.
* Working and liaising with other agencies.
