RGN required for Social Inclusion Community Healthcare East Clonskeagh Hospital, Dublin 6
1. Full time hours
2. RGN’s with some experience within social inclusion or work with marginalised groups. A self- starter and an independent worker.
3. Initiate nursing assessment and prepare nursing care plan, implementing and evaluation of same for all SI groups Travellers, Ukrainians, International protection applicants, Homeless persons and all SI marginalized groups.
4. Prioritize workload and situations as appropriate and work closely with the DON.
5. Staff nurse in clinical settings / outreach clinics as required.
6. Staff nurse in the sexual health clinics of the Gay Men’s Health service, Women’s Health service or Anti Human Trafficking service when required. (this is not the primary focus of this post)
7. Work with the DON and assist with the development of new, innovative and progressive Nursing projects for all SI cohorts and implement same
8. Promote equality and inclusivity amongst all SI cohorts LGBTQ+, Domestic Sexual Gender Based Violence, Traveller and Roma communities, International protection applicants and Ukrainians
9. Maintain good collaborative working relationships and communications with appropriate statutory, professional and voluntary organisations responsible for and /or participating in health care.
10. Attend clinical/ management supervision with the DON
11. Support Primary Health Care Traveller Projects (PHCTP) and SI services.
12. Work and liaise with the PHCTP, IPAS centers staff, Community Health Networks, community services and NGO’s supported by the Social Inclusion Team to ensure integration and optimum healthcare access.
13. Work and collaborate with social care staff regarding patient care.
14. Support the delivery of National Screening Initiatives for the Travelling community Cardiovascular, Pulmonary, Metabolic disorders as required.
15. Work as part of a multidisciplinary team to meet the needs of the clients and their families, advocating on their behalf as necessary.
16. Respect the dignity and confidentiality of clients and their families
17. Have knowledge of Health Service Executive – CHEast policies and procedures and observe same.
18. Be informed of Nursing Policies and Procedures and An Bord Altranais guidelines.
19. Manage incidents in accordance with policy and function quickly and effectively in emergency situations
20. Contribute to nursing quality assurance and risk assessments by assisting with the identification of recurring nursing problems and co-operate in data collection.
21. Adhere to the standard system of documentation and evaluation of nursing care.
22. Maintain high standard of clinical, professional and ethical responsibility.
23. Promote a safe environment for clients, families and staff.
24. Participate in the audit of clinical care.
25. Participate on groups, committees; attend team meetings as required.
26. Participate in the development of policies and procedures and SOPs that will maximise efficiency of the service provided to the enhancement of client care.
27. Contribute to the development and implementation of information-sharing protocols, audit systems, referral pathways, individual care and shared-care arrangements.
28. Participate in the design and delivery of Education/Training initiatives to SI services / clients / Traveller Health Projects
29. Act as a mentor and coach to projects/ SI services in the implementation of Health Care Programmes.
30. Maintain optimum standards through the use of relevant educational opportunities.
31. Take measures to develop and maintain the competence necessary for professional practice.
32. Participate in ongoing in-service training in order to keep up to date with current issues and maintain competencies.
33. Act as a resource person and catalyst to enable other health care workers to work effectively to achieve positive health outcomes.
