The position of Home Support Resource Manager encompasses both managerial and administrative responsibilities.
The HSE Home Support Service (formerly called the Home Help Service or Home Care Package Service) aims to:
· Support older people to remain in their own homes for as long as possible
· Facilitate timely discharge home from the acute setting
· Reduce readmission to the acute setting
· Support informal carers
· Support carers so that they might be able to continue to provide care for older people.
The support provided is based on identified needs and may be provided by the HSE or by an external provider, approved by the HSE.
The service is primarily provided to people over the age of 65 who need support to continue living at home however, sometimes exceptions are made for those younger than 65 who may need support
Purpose of the post:
Manage the delivery of Home Support Services to clients within a defined geographical area Manage a team of Healthcare Assistants (Home Support) (HCAs) in the specific geographical area to which he/she has been assigned Work closely with members of the local primary care teams, Integrated Care Programme for Older Persons teams, Enhanced Community Care teams and external service providers to ensure that services are delivered in an integrated manner We welcome enquires about the role.
