We have an opportunity for a Clinical Case Manager to join our Outpatient team.
As a Clinical Case Manager, you will work collaboratively with a multidisciplinary team to provide outpatient mental health services and community support. You will be responsible for developing and implementing treatment plans that are tailored to the individual needs of our clients. Our Case Managers work closely with adults who have severe and persistent mental illness to identify goals and develop strategies for success. This includes identifying and utilizing community resources, liaising with other healthcare providers, referring clients to relevant programs, and providing life skills training.

Work Schedule:
The standard work schedule is Monday-Friday, 9:00AM-4:00PM.
Responsibilities:
* Advocate for clients' needs, including medical appointments, welfare and vocational programs, and legal systems.
* Provide comprehensive community support to an assigned caseload.
* Conduct individual therapy sessions as needed to address client goals and treatment planning.
* Visit clients in their homes to assess living situations and provide treatment interventions when necessary.
* Facilitate group therapy sessions and organize special events for clients.
* Assist with discharge planning and create written summaries of treatment as needed for client referrals and releases of information.
* Evaluate treatment effectiveness with the agency's medical and clinical team, revising treatment documentation as required by law and per supervision direction.
* Maintain accurate progress notes according to agency guidelines.
* Attend scheduled meetings and conferences, including regular meetings with the Clinical Director and/or Psychiatrist.
* Provide immediate crisis intervention and resolution as needed.
* Monitor medication distribution as necessary.
* Maintain confidentiality of client and staff information at all times.
* Perform other duties as assigned.
Qualifications:
* Bachelor's degree in social sciences or a related field.
* At least 6 months of clinical experience working with individuals with severe mental health and/or substance use disorders.
* Certification from the Addiction Counselor Association (AAC) and ability to obtain certification within 30 days of hire.
* Current CPR/First Aid certification or ability to obtain it within 30 days of hire.
* Legible handwriting.
* Able to perform basic arithmetic calculations.
* Able to read and understand client charts.
* Holds a valid Washington State driver's license for one year with a clean driving record to operate agency vehicles.
* Prior experience working with mental health populations.
* Able to perform duties with reasonable accommodation.
