Job Title: Housing First Case Manager
Cpl Healthcare, in partnership with our client, a non-profit organisation based in Dublin, Ireland, provides services for individuals who are homeless and at risk of homelessness in Ireland.
Job Summary:
This role involves working as part of the Housing First Case Management Service to deliver a 'Housing First' approach to individuals experiencing long-term homelessness in County Clare. The successful candidate will work closely with clients to establish trusting relationships, provide support with housing, welfare rights, and entitlements, and assist them in achieving independent living.
Key Responsibilities:
* Work to the principles and fidelity of the Housing First approach.
* Provide a safe and welcoming environment for clients, respecting their choice and self-determination.
* Establish trusting relationships with clients, especially those who are living on the streets or in temporary accommodation.
* Undertake assessments and support planning with clients to help them progress toward independent living.
* Develop methods and structures to support clients with their individual housing, welfare rights, and entitlements.
* Assist tenants in creating and sustaining a home, implementing practices aligned with tenancy settlement indicators.
Requirements:
* Qualification in social care, youth work, or a related discipline at degree level.
* Experience of working with customers who are or have been homeless.
* Experience of working with customers identified as having a dual diagnosis.
* Team working experience.
* Report writing abilities and IT skills.
* Experience of working and liaising with other agencies.
