About the Role:
As a Client Care Manager, you will be responsible for assessing client needs, developing personalised care plans, and coordinating care services to enhance their well-being. You will work closely with clients, families, caregivers, and healthcare professionals to provide exceptional support and ensure care plans are delivered to the highest standards.
Key Responsibilities:
1. Conduct comprehensive assessments of clients’ needs and create tailored care plans in collaboration with families and healthcare teams.
2. Oversee and coordinate the implementation of care services, ensuring compliance with regulations.
3. Regularly review and adjust care plans to meet evolving client needs.
4. Provide guidance and training to caregivers, ensuring they have the necessary resources to deliver quality care.
5. Act as the primary point of contact for clients and their families, addressing concerns in a compassionate and timely manner.
6. Work closely with an interdisciplinary team to deliver holistic, person-centred care.
7. Maintain accurate records of client assessments, interactions, and care plans.
What We’re Looking For:
1. QQI Level 6 in Health Services Supervisory Management
2. Experience in a healthcare or homecare setting
3. Strong communication and interpersonal skills to build trust with clients and families
4. Excellent organisational and problem-solving abilities
5. Ability to work independently and collaboratively in a dynamic environment
6. Proficiency in Microsoft Office and electronic health record systems
7. Full driver’s license and access to a reliable vehicle (preferred)

Work Schedule:
