Kildare Immediate start required The role of Clinical Care Coordinator (CNM2) incorporates the skills of a nurse, a case manager, Team Leader, and a patient advocate.
The position requires a knowledge of evidence-based care, managing complexity and end of life care, quality improvement and organisational skills.
The Clinical care Coordinator will: Participate in managing (planning, co-ordinating, tracking and evaluating) referrals for Short stay beds across the CHO in conjunction with referrers, care providers, clients, and families.
Be responsible for clinical data management.
Track the frail Older Person across acute and community sectors and collate and report on short stay bed referral/ Activity data.
The CNM2 will have a pivotal role in: The coordination and sourcing of appropriate Short Stay Respite (SSR)care for our older persons population across the DSW/KWW CHO The triaging and coordination SSR referrals from a clinical perspective submitted to the CHOs Short Stay Bed central referral inbox .This will be done in conjunction with the Short Stay Bed Administrator The provision of ongoing reviews and case management The undertaking of other Clinical Duties as deemed appropriate Participating in the Local Placement Forum (LPF) Meetings as required Providing Clinical input and expertise to care needs and care planning of Complex Cases For further information please forward your CV via the link provided: For more information call Sorcha on or email your CV to Skills: communication confidence teamwork
