HSE Mid-West Disability Services focus on enabling people with disabilities to achieve their full potential, as independently as possible while ensuring that their voices and that of their families are heard.
HSE Mid-West Disability Services, encompasses services for children, young people and adults who have an Intellectual Disability, Physical and Sensory Disability, Autism or Acquired Brain Injury.
It also includes management/monitoring of Service Level and Grant Aid Agreements through which Section 38/39 and Private Providers are funded.
It encompasses development and oversight of new and emerging service delivery models.
Services are delivered through the model of health and social care include the following:
· Children Disability Network Teams
· Respite and Residential placements
· Home Support Services
· Outreach services
HSE Children's Disability Case Management Service is responsible for the planning, arranging and subsequent co-ordination and delivery of services using individually tailored care plans, having a person-centered approach and a multi-disciplinary focus.
The person appointed to this post will be required to participate as a member HSE Children's Disability Case Management Team delivering a coordinated approach to client care.
S/he may be required to work as a key worker for particular cases as required.
