CNM II Patient Flow/Long Term Care Coordinator

PURPOSE OF THE ROLE
1. The CNM2 will be responsible for the management of all patients referred for long term care from Tallaght University Hospital.
2. Have a responsibility in managing the long term care applications process with patients and their relevant person.
QUALIFICATIONS
Each candidate must meet the following qualifications by the latest date for receiving completed application forms:
1. Be registered in the General Division of the active Register held by Nursing and Midwifery Board of Ireland or be entitled to be so registered.
2. Have completed a degree course to level 7 or higher in the NFQ framework.
3. Have 5 years post registration experience and a minimum of 2 years in the specialty or related area.
4. Have the clinical, managerial and administrative capacity to properly discharge the functions of the role.
5. Have excellent communication and interpersonal skills.
6. Demonstrate evidence of continuing learning and professional development.
Desirable Criteria
1. Evidence of management experience in an acute hospital setting.
2. Evidence of knowledge of long term care process in the hospital.
3. Experience working in patient flow.
2. Age Restrictions In Relation To Applications
Age restriction shall only apply to a candidate where he/she is not classified as a new entrant (within the meaning of the Public Service Superannuation (Miscellaneous Provisions) Act, 2004). A candidate who is not classified as a new entrant must be under 65 years of age on the first day of the month in which the latest date for receiving completed applications for the office occurs.

A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health as would indicate a reasonable prospect of ability to render regular and efficient service.

5. Garda Vetting
Garda vetting is sought for all employees and prospective employees of Tallaght University Hospital. Given the specialised nature of the services provided, your appointment will be subject to satisfactory Garda Vetting and re-vetting in circumstances where the Hospital deems it appropriate.
6. Mandated and Designated Persons under Children First Act 2015
All mandated persons have two main legal obligations under the Children First Act 2015. They are required to report any knowledge, belief or reasonable suspicion that a child has been harmed, is being harmed, or is at risk of being harmed. They are also required to assist Tusla, if requested, in assessing a concern which has been the subject of a mandated report.
PARTICULARS OF OFFICE
The appointment is permanent, full time and pensionable.
Remuneration:
Remuneration is in accordance with the salary scale approved by the Department of Health & Children. Current remuneration effective from 1st June 2024: €59,070 by 11 increments to €74,643 incl. 1 LSI.
4. Duties:
The CNM2 will perform such duties as are outlined in this job description.
5. Hours of work:
Normal working hours are 37.5 worked over 5 days. Your contracted hours of work are liable to change between the hours of 8 am to 8 pm over 7 days to meet the requirements for extended day services.
6. Probation:
The successful candidate will be appointed initially for a probationary period of 6 months.
7. Annual Leave:
Annual leave entitlement is 25 -28 working days (pro rata), depending on length of service, plus 10 Bank Holidays per annum.
8. Sick Leave:
Payment of salary during illness will be in accordance with arrangements as approved from time to time by the Department of Health and Children.
9. Termination of Office:
The employment may be terminated at any time by 2 months’ notice on either side except where dictated by the Minimum Notice and Terms of Employment Act 1973/91.

1. Maintain throughout the Hospital an awareness of the primacy of the patient in relation to all Hospital activities.
2. Ensure a high standard of care delivered safely in accordance with the hospital policies and procedures.
3. Build good relationships with key stakeholders in the patient’s journey through hospital.
4. Demonstrate knowledge of the Hospital/Departmental emergency plans.
5. Ensure the provision of a high standard of care to the patient and families.
6. Demonstrate behaviour consistent with the Values of the Hospital.
7. Commitment to continuous professional development.
8. Reporting relationship will be to the CNM 3 Patient Flow/Unscheduled Care Manager.
9. Be professionally accountable to the Director of Nursing.
SPECIFIC ACCOUNTABILITY
Managing the Service - Quality & Safety of Service / Delivery of Results (level 3)
1. Ensure each patient on the list has a completed InterRAI or Common Summary Assessment Report completed within the agreed timeframe.
2. Complete chart review to ensure Age Related Health Care has documented that patient is for Long Term Care.
3. Manage under 65 year old Long Term Care listings.
4. Refer suitable patients to transitional beds as appropriate.
5. Participate at local placement forum when required.
6. Meet with Social workers to obtain updates on financial and legal side of application.
7. Be aware of the Fair Deal timeframes.
8. Ensure timely and clear communication with patients and their families regarding the long term care process.
9. Develop knowledge of nursing homes.
10. Develop good working relationships with public and private nursing homes.
11. Liaise closely with MDT in particular OT for ongoing needs.
12. Escalate any delays in processes to patient flow manager.
13. Keep up to date with any developments in the area of Long Term Care.
14. Ensure bed managers and Patient Placement Coordinator are aware of date of discharge.
15. Update ward managers with discharge plan.
16. Management of the Delayed Transfer of Care list for all LTC patients.
17. Ensure Synergy is updated in a timely manner regarding all LTC patients.
18. Maintain a database for all patients referred to LTC.
19. Liaise closely with Consultants regarding updates.
Discharge Planning
1. Assist the Discharge Planning Team and Bed Management.
2. Liaise with the CNM’s, Doctors and other members of the discharge planning team.
3. Perform morning ward round to identify discharges.
4. Assist in the identification of patients who are fit for discharge.
5. Communicate with relevant members of the Multi-Disciplinary Teams on a daily basis.
6. Regular communication with Consultants if discharge issues arise.
7. Assist in identifying process flow delays.
8. Have some knowledge of long term care, delayed and complex discharges.
Discharge policies
1. Participate in the Discharge Planning group.
2. Implement and monitor Hospital Bed Management policies.
3. Review the effective functioning of all policies relating to Discharge Planning.
4. Promote awareness of Bed Management and discharge policies throughout the Hospital.
5. Facilitate appropriate transport for patients on discharge.
Service Quality
1. Support the Bed Manager, Discharge Planning Team and clinical teams.
2. Be aware of the importance of understanding the IPC status of patients.
3. Maintain a record of clinically relevant data.
4. Participate in Quality Improvements Initiatives/Programmes.

1. Act as a change agent for staff.
2. Facilitate ongoing development of nursing documentation.
3. Liaise with all registered nurses and HCAs.
