Overview of the Role
The post holder will be responsible for providing a service that optimizes safe, high-quality, and holistic care. They must maintain a high standard of clinical practice, ensure patient confidentiality and privacy, and act as an advocate for patients with acquired brain injury and stroke.
Supporting NRH Interdisciplinary Teams (IDT) in rehabilitation planning for patients is also a key responsibility.
Accountability, Duties, and Responsibilities
The post holder has professional and legal accountability for decisions made, which impact patient and staff health, safety, and welfare. Ensuring a safe environment for staff and visitors, in compliance with Health and Safety requirements and best practices, is essential.
Maintaining day-to-day security of the work area, being aware of fire regulations and security arrangements, and keeping up to date with mandatory training are also critical responsibilities.
Specific Accountability
The post holder is accountable for ensuring a high standard of patient care, including maintaining patient privacy and dignity at all times. They must manage their role as an outreach specialist within the Brain Injury and Stroke Programmes and maintain their own case load.
Linking with other professionals, as necessary, to meet all patients' needs is also required.
Outline of Duties and Responsibilities
Acting as a support and liaison between NRH and referring agencies, in the interest of the patient with complex needs, is a key duty.
Providing assessment and recommendations for patients and families, as required, and gaining high-quality pre-admission assessments of rehabilitation needs are also essential tasks.
Communicating rehabilitation needs to NRH IDTs to assist with rehabilitation planning and providing recommendations for the best placement along the rehabilitation continuum are critical responsibilities.
Providing direct patient input and IDT education to support admission or post-discharge rehabilitation, coordinating IDT outreach or other visits, and attending waiting list management meetings are also required.
Management
Maintaining accurate records, including statistical data relevant to patient care, and submitting activity data as required are key responsibilities.
Managing caseload effectively, establishing links with other organizations, and representing the hospital at local, national, and international meetings, as required, are also critical duties.
