Post Title: Complex Discharge Department Manager Post Status: Permanent ContractDepartment Complex Discharge Department Location: Beaumont Hospital, Dublin 9 Reports to: Deputy Head of Patient AccessSalary: Appointment will be made on GRADE VII – CLERICAL (€59,419 - €77,243 LSI) at a point in line with Government pay policy. Position Summary: The complex discharge office is a vital division of Corporate Operations. Working in close collaboration with the Patient Flow Team and Multidisciplinary Teams the Complex Discharge Department Manager is an essential part of the patient pathway.
Responsibilities
Complex Discharges Manage the complex discharge process for patients in Beaumont HospitalLiaise with community services and stakeholders in the complex pathwaySupport clinical teams and ward manager in the management of patients with complex discharge needsParticipate in Care Planning Meetings at ward levelManage the funding streams related to complex dischargesManagement and oversight of the Business Intelligence Unit (BIU) ensuring all delayed transfer of care is reported accordingly Rehab Pathway Management Establish a central process for the recording pf all patients for rehabilitationMaximise rehabilitation opportunities across all off site facilitiesCollaborate with Health and Social Care professionals Stakeholder Engagements Build relationships with community partners to develop new pathways and ensure efficiency of existing pathways to off-site facilitiesCollaborate with multidisciplinary teams to develop and manage efficient discharge pathwaysCentralise management of complex and protracted discharges across the hospitalLead on the planning and review of patient with a Length of Stay >10daysLead on meetings with community partners in the Dublin North and North East Area Education and Teaching Actively participate in education and training of people within and external to the complex discharge officeParticipate in education programmes for clinical and non-clinical staff around the complex discharge processes Quality and Performance Prepare and report on performance metrics related to the departmentEnsure actions are planned and executed to address performance issuesEstablish new and innovative ways of meeting the hospitals strategic prioritiesParticipate in accreditation programmes including JCI, OECI etcProactively lead and contribute to the development/implementation of best practice service modernisation and improvement plansEnsure policies, procedures and guidelines relating to the office are developed, adhered to and evaluatedEncourage a culture of continuous improvement and setting/supporting achievement of the highest possible standards with maximum People Management Develop and manage the complex discharge teamActively promote and develop a positive working environment, ensuring a highly focused/skills/educated/motivated Maximise the contribution of these staff by developing systems for performance appraisal, personal development, individual and organisational development and continued skill mix Ensure that the responsibilities and objectives for direct reports are clearly defined and understood In conjunction with the Head of the department, regularly reviewing staff complements and skill mix across the department to ensure that staffing is appropriate for current and projected clinical and operational Be responsible for managing sickness and absence in accordance with hospital Act as a point of contact for staff on human resource issues in accordance with HR policies and Be responsible for facilitating all staff to continue their education and personal development to support improved organisational Ensure the involvement of all staff through open and direct communication to sustain a high level of commitment and flexibility from the department workforce through implementing the hospital Communication Strategy and in particular the team briefings Promote equal opportunity and adherence to best practice employment and approaches to managing and implementing
Qualifications
Mandatory: At least 5 years’ experience in an Acute Healthcare setting Management qualification Proven track record in complex discharges Desirable: Working knowledge of process improvement methodologies and approaches Knowledge of social services and voluntary organisations.
