Knowledge, skills and training
Essential criteria
1. Registered and valid Mental practitioner Nurse/OT/Social Worker
2. Evidence of continuous professional development
Desirable criteria
1. Evidence of working with patients with a dementia diagnosis and older persons
2. Evidence of working in the community
Job Specific Experience
Essential criteria
1. Able to assess, plan, implement and evaluate programmes of care.
2. Risk assessment and management
3. The ability to prioritise and meet deadlines; organise work of self and others; supervising, teaching and assessing; self-motivated with the ability to motivate others; effective resource management.
4. Uses initiative and can make decisions. Manages difficult situations effectively.
