Role Purpose:
As a Social Worker, you will provide essential emotional and practical support to patients across all wards. Working in close collaboration with a multidisciplinary team, you’ll play a key role in guiding patients through their hospital journey, offering counselling, discharge planning, and advocacy services to ensure they have the resources and support needed for their recovery.
Key Responsibilities:
1. Patient Support: Deliver compassionate counselling, information, and support services tailored to the needs of patients and their families.
2. Social Work Services: Conduct thorough assessments, assist with discharge planning, and connect patients with community resources to ensure a seamless transition from hospital to home.
3. Multidisciplinary Collaboration: Act as a consultant to our healthcare team on the psychosocial aspects of patient care, arranging case conferences and working closely with medical, nursing, and allied health professionals.
4. Community Liaison: Partner with community health services and other agencies to guarantee continuous care and support for patients beyond their hospital stay.
5. Professional Development: Engage in ongoing professional education, maintain required registrations, and attend workshops and conferences to stay informed on best practices.
6. Communication: Keep accurate and detailed patient records, share relevant information with the healthcare team, and contribute positively in team meetings to support a collaborative environment.
Qualifications:
1. Essential: National Qualification in Social Work (NQSW)
2. CORU registration.
3. Ideally have Medical Social Work experience but not essential
4. Skills: Proficiency with IT systems, including Windows and Office software.
