Key elements of the role are the delivery of evidence-based practice for patients presenting with an immediate nursing need or a long-term condition and the provision of preventative health care to the practice population.
They will demonstrate safe clinical decision-making and provide expert care in the management of patients.
The post holder is expected to work collaboratively with all members of the practice team to meet the needs of patients and support practice objectives, policies and procedures.
The post holder will work within their scope of practice and in accordance with local policies and procedures and the NMC Code of Professional Conduct.
Key ResponsibilitiesA.
Clinical Assist in and perform routine tasks related to patient care as indicated by patient need including:CytologyRemoval of Sutures/clipsTravel ClinicAsthma and COPD ManagementVenepunctureHypertension Management / 24-hour ambulatory BPDiabetes ManagementEar syringingRoutine immunizations/Childhood immunisationsChaperoning and assisting patients where appropriate who are being examined by another clinicianRequesting pathology tests, for example, urine culture, swabsFollowing agreed clinical protocols.Maintaining and cleaning equipment used by the nurses and GPs and upholding the standards required for Infection prevention and control.Maintaining GP and Nurses rooms, stocking and rotating items as requiredMaintain general tidiness and cleanliness of nurses and treatment roomsSupervision of the HCAsOrdering of Health Promotion LiteratureOrdering of vaccinations to maintain stock levels if requiredParticipation in administrative systems in the practiceMaintain accurate recordsAdvise other team members of potential problems or errors within the range of assigned tasksAttend and participate in any practice/clinical meetings monthly.On occasion there may be a requirement to undertake home visitsSupport and manage the QoF programmes and income generation for the practice relevant to roleAny other delegated duties appropriate to the postB.

Managing information Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating informationReview and process data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processesManage information searches using the internet and local library databasesUnderstand responsibility of self and others to the practice and primary care trust regarding the Freedom of Information ActH.
