Job description We seek a dedicated Clinical Care Manager to lead and deliver person-centred care, ensuring service users preferences, values, and needs are prioritized while following home support best practices.
Key Responsibilities:
Care Planning:
Oversee and collaborate with Supervisors to develop tailored, person-centred care plans.
Service User Autonomy:
Empower service users to make informed decisions and maintain independence.
Staff Supervision & Training:
Mentor and equip staff with person-centred care principles.
Quality & Safety:
Monitor care outcomes and lead quality improvement initiatives.
Collaboration:
Work with a multidisciplinary team to ensure holistic, seamless care.
Advocacy:
Champion the rights and voices of staff and service users.
Qualifications & Experience:
Licensure: Registered with the Nursing and Midwifery Board of Ireland (general division).
Experience: 5+ years post-registration, with 2+ years in a community-based role.
Additional:
Management/home support experience desirable; willingness to undergo further training.
Requirements:
Full driving licence, own car, IT proficiency in care management system.
