Consultant Physician in Geriatric Medicine The HSE Community Health Organisation Dublin North City & County and Beaumont Hospital are seeking to appoint an enthusiastic, dynamic Geriatrician with excellent leadership and service development skills. This new substantive post has been developed to support the expansion of the existing integrated care team for older people (ICPOP), Dublin North. The appointee will become a fully integrated member of the Beaumont Geriatric Medicine Department and will lead a team of dedicated healthcare professionals in provision of medical services to community-dwelling older people with complex needs in the North Dublin area. The Geriatric Medicine Department The appointee will join 10 existing consultants in a well-established Geriatric Medicine Department. The Department provides a wide range of inpatient, outpatient and liaison services. Services offered include acute inpatient care on two Specialist Geriatric Wards, orthogeriatrics, fracture liaison, 58 off-site rehabilitation beds, Emergency Department liaison, community integrated care, stroke (National Thrombectomy Service), a 5 day ‘Day Hospital’ service, and 6 outpatient clinics. The Department supports an approximately 1/30 GIM roster. While the Department supports the stroke service this post has no commitment to the stroke and thrombectomy rosters. ICPOP RoleThe Integrated Care Team for Older People (ICPOP) provides specialist geriatric assessment and interventions using a multidisciplinary approach to support older people with complex care needs living in their own homes. ICPOP provides support and education to the older person, carers and healthcare professionals involved in managing their care, in the acute and community setting and enables more robust communication across sectors. The team consists of one ANP, one CNS, one physiotherapist, two OTs, 0.5 wte SLT, 0.5 dietician and 0.5 consultant geriatrician input. ICPOP supports appropriate and timely reduction of Emergency Department (ED) attendance and admissions, through the development of care pathways that support GPs and healthcare professionals who assess and manage older people with escalating care needs. The ICPOP consultant provides support and clinical direction to the Emergency Department (ED) Frailty Intervention Therapy Team (FITT) in the assessment and management of older patients. This new consultant post is essential for further expansion and implementation of pathways and services in community settings as alternatives to acute hospital care. The post will enable greater activity of the ED liaison service by supporting the FIT team in assessing older people who present acutely as well as the community AHPs within the ICPOP and local GPs in their management of older people with complex needs. There is scope within this role to develop subspecialties as per the applicant’s skills and interests. Beaumont Hospital Beaumont Hospital, incorporating St. Joseph’s Raheny, is a large academic teaching hospital 5km north of Dublin City centre. It is a Level 4 hospital with its academic partners The Royal College of Surgeons in Ireland University of Medicine & Health Sciences and Dublin City University. Beaumont Hospital provides emergency and acute care services across 54 medical specialties to a local community of some 290,000 people. In addition, it is a designated Cancer Centre and the regional treatment centre for Ear, Nose & Throat, and Gastroenterology, the national referral centre for Neurosurgery and Neurology, Renal Transplantation, the National Poisons Information Centre, Cochlear Implantation and Mechanical Thrombectomy. The Hospital has been designated as the European Stroke Organisation (ESO) Stroke Centre in addition to the National Centre for Skull Based Cancer Services. With a workforce of approximately 4,500 staff and a combined bed complement of 1058 (IP & DC) beds, Beaumont Hospital is the principal teaching hospital for the Royal College of Surgeons in Ireland. It also enjoys close links with Dublin City University, especially in the area of nurse training, and with other academic institutions in respect of training and research. The St. Joseph's Hospital Raheny site provides both medical and surgical inpatient care, day-care, outpatient, outpatient physiotherapy, radiology services, Care of the Elderly Rehabilitation and a 100 bedded Raheny Community Nursing Unit.
