Candidate/Registered Advanced Nurse Practitioner - Community and Acute Services, Specialist Palliative Care

* Located at Marymount University Hospital & Hospice in Curraheen, Cork and other locations as required.
This role will integrate with community palliative care and assist in responding to the most complex referrals to the Community Palliative Care team in a timely manner, thereby improving key performance indicators for the community. This specialist role aims to bridge the gap between hospital-based and community-based services, ensuring that patients receive person-centred, holistic care regardless of their setting.
The ANP will act as a key liaison between acute hospitals, hospice teams, community healthcare providers, and primary care teams to streamline complex discharges and ensure that patients receive timely, appropriate palliative care interventions. This will include specialist symptom assessment and management. The ANP will prescribe and adjust pharmacological and non-pharmacological interventions to optimise comfort and quality of life. Additionally, the ANP will undertake complex communication processes such as advanced care planning and, where appropriate, employ chronic disease management skills, including supportive self-management education, while enabling and supporting carers and staff. The ANP will work collaboratively with palliative medicine consultants, fellow ANPs in acute and community services, pharmacists, and multidisciplinary teams to ensure evidence-based symptom management approaches.
The ANP will also support the upskilling of nursing staff, junior doctors, allied health professionals, and carers in best-practice palliative care approaches. This role involves developing and delivering training programmes on topics such as pain management, communication skills, advance care planning, and end-of-life symptom control. The ANP is required to promote a culture of continuous learning and excellence in palliative care delivery.
This role is instrumental in enhancing patient experience, streamlining care transitions, and building capacity within healthcare teams. By ensuring holistic, compassionate, and evidence-based palliative care interventions, the ANP will play a crucial part in improving the quality of life for patients and families navigating serious illnesses.
Eligibility Criteria
* Be registered in the General Division of the Register of Nurses, as maintained by the Nursing and Midwifery Board of Ireland.
* Hold a third-level qualification in palliative care and be willing to undertake an MSc in Advanced Nursing Practice.
* Have seven years of relevant post-registration experience, including a minimum of five years in specialist palliative care.
* Possess the clinical, managerial, and administrative capacity to effectively discharge the responsibilities of the role.
* Demonstrate evidence of continuous professional development.
* Exhibit excellent communication and interpersonal skills.
* Be proficient in the use of Microsoft applications (e.g., Excel, Word, PowerPoint).
Desirable Criteria
* Possession of an MSc in Advanced Nursing Practice or a closely related field.
* Proficiency in utilising electronic patient record systems.
