**About the Role:**
The purpose of this role is to undertake individual work with families and customers who face chronic exclusion due to their many complex and overlapping needs, utilising an Intensive Care and Case Management Model.
Work collaboratively within an intensive family therapeutic support model, assessing needs, advocating on behalf of clients, providing support and making appropriate referrals.
Work within a five-day service, Monday – Friday 9-5. Flexibility may be required from time to time to meet the needs of the service.
Responsibilities:
* Engage families with individual needs, considering dynamics relevant to their needs and behaviours.
* Develop and maintain links with Health Service Executive, Tusla, Probation, and other clinical, community, and statutory services.
* Develop community links with accommodation and service providers.
* Effectively engage with all stakeholders in the child's, parent's, and customer's lives.
* Contribute to a case conferencing model, ensuring effective liaison with external professionals and the family.
* Provide one-to-one engagement to support families in making informed decisions and performing an intensive in-depth family support model to agreed standards.
* Implement education programmes, using coaching and mentoring interventions to assist parents grow in life and parenting skills.
* Contribute to formulating customers' needs assessments and support plans, and keep up-to-date case work records.
* Identify significant needs/behaviours in clients' lives, addressing issues contributing to homelessness.
Qualifications and Experience Needed:
* Degree in Social Care or similar discipline.
* 2 years experience working with high-need families at high risk utilizing an intensive case management and family support model.
* Understanding of issues pertinent to high-need families with chronic child welfare issues and homelessness.
* Knowledge and understanding of children, adolescents, and issues affecting parents.
* Experience working with customers with co-existing conditions, team working, liaising with agencies, and computer skills.
* Effective communication and report writing abilities.
