Position Summary:Validation is a continuous process, to ensure that waiting list information is accurate, up to date, and identifies patients that are willing to proceed with hospital care.Clinical Validation is the process whereby the Clinical Nurse Manager conducts a review of the patient’s healthcare records and/or medical notes/ referral letters. This post is being developed in response to national and local service plans, and local service need as evidenced by the growing patient population requiring endoscopy services.The purpose of this post is to carry out Clinical Validation of the National P2 (Routine / Non-Urgent /Surveillance) Endoscopy Waiting List. This post is required to ensure accuracy of the current waiting list. The successful candidate will be responsible for safely removing those patients who no longer require a procedure from the waiting list. This maximizes the efficient use of slots available for current endoscopy patients and reduces wait times for routine scope lists.The Endoscopy Department in Beaumont Hospital is a JAG Accredited unit consisting of 4 endoscopy procedure rooms. Elective and inpatient procedures are carried out Monday to Friday in the Endoscopy Unit.The unit carries out the following procedures: Colonoscopy, OGD, Sigmoidoscopy, EUS, ERCP, EBUS & Bronchoscopy.
Responsibilities
Professional / ClinicalClinical Focus CNM2 Clinical Validation Endoscopy will have a strong patient focus whereby the specialty defines itself as nursing and subscribes to the overall purpose, functions and ethical standards of Nursing. The clinical practice role may be divided into direct and indirect care. Direct care comprises the assessment, planning, delivery and evaluation of care to the patient, family and/or carer. Indirect care relates to activities that influence and support the provision of direct care. The Clinical Nurse Manger 2 Clinical Validation Endoscopy will: Direct Care• Provide a specialist nursing service for patients awaiting endoscopic procedures and support decisions on correct treatment pathways for the continuum of care. • Undertake comprehensive patient review of endoscopy referrals based on triage proforma for scheduled endoscopic procedures• Use the outcomes of patient review to develop and implement pathway of care/case management in conjunction with the multi-disciplinary team (MDT) and the patient, family and/or carer as appropriate.• Make alterations in the management of patient’s condition in collaboration with the MDT and the patient in line with agreed pathways and policies, procedures, protocols and guidelines (PPPG’s).• Co-ordinate agreed alternative pathways of care with the Consultant, waiting list officers, and GP as appropriate. • With support of the waiting list officers communicate with patients, family and /or carer as appropriate. • Where appropriate work collaboratively with MDT colleagues across Primary and Secondary Care to provide a seamless service delivery to the patient, family and/or carer as appropriate.• Identify health promotion priorities for the patient, family and/or carer and support patient self-care in line with best evidence. Indirect Care• Validation management- submit monthly validation statistics and invoices to the NTPF as per the agreed targets for the post.• Identify and agree appropriate pathways of care / SOP’s for patients referred for endoscopy procedures post validation.• Participate in case review with MDT colleagues if required• Take a proactive role in the formulation and provision of evidence based PPPGs relating to endoscopy validation • Take a lead role in ensuring the service for patients referred for validation is in line with best practice guidelines. Patient/Client Advocate • Communicate, negotiate and represent patient’s family and/or carer values and decisions in relation to their condition in collaboration with MDT colleagues in both Primary and Secondary Care as appropriate. • Develop and support the concept of advocacy, particularly in relation to patient participation in decision making, thereby enabling informed choice of treatment options• Respect and maintain the privacy, dignity and confidentiality of the patient, family and/or carer. • Establish, maintain and improve procedures for collaboration and cooperation between Acute Services, Primary Care and Voluntary Organisations as appropriate.• Proactively challenge any interaction which fails to deliver a quality service to patients. Clinical Nurse Manager 2 Clinical Validation Endoscopy. Education & Training• Develop clinical competence in validation processes and become familiar with the patient management systems used within the endoscopy unit to include the clinical validation app in PAMS (CVAL). • Be familiar with the Endoscopy Clinical Validation Operational Guidance Including minimum Invoicing information requirements. • Create exchange of learning opportunities within the MDT in relation to evidence based practice and validation processes.• Be responsible for addressing own continuing professional development needs. Audit & Research• Maintain and update the validation register with the support of the waiting list officers. • Maintain a record of clinically relevant data aligned to National Key Performance Indicators (KPI’s), JAG Accreditation requirements and wider hospital data requirements. • Identify, initiate and conduct nursing and MDT audit and research projects relevant to the area of practice.• Identify, critically analyse, and disseminate evidence based data on validation practice. • Contribute to nursing research on all aspects of validation practices • Use the outcomes of audit to improve service provision.• Contribute to service planning and budgetary processes through use of audit data and specialist knowledge.• Monitor, access, utilise and disseminate current relevant research to advise and ensure the provision of informed evidence based practice. Audit expected outcomes including• Collate data which will provide evidence of the effectiveness of Clinical Nurse Manager 2 (validation) involvement.• Meet agree weekly validation targets as determined by the NTPF. • Evaluate audit results and research findings to identify areas for quality improvement in collaboration with nursing management and MDT colleagues (Primary and Secondary Key working relationships • Consultant Gastroenterologists• Waiting List Office and Scheduling Staff• External Endoscopy Colleagues Patient Advocate• Provide and efficient, effective and high quality service, respecting the needs of each patient• Acts as an advocate for patients providing information as appropriate and communicating with relevant staff in the Endoscopy Unit• Participate in team discussions regarding treatment for patients Consultancy• Provide leadership in clinical practice and act as a resource and role model for practice• Generate and contribute to the development of clinical standards and guidelines, having regard to national and international advancements in Gastroenterology nursing• Establish collegial partnerships and in the context of Gastroenterology/Endoscopy, contribute to the professional development of students, peers, colleagues and others through consultation, education, leadership, mentorship and coaching• Work with, support, advise and help build up knowledge of expertise of the primary care team in providing an appropriate system of care to meet the needs of the patient and their family/carers• Develop and maintain good collaborative working relationships and communication with all members of the speciality’s MDT and other health care professionals within and outside the service• Participate in the implementation of the national clinical care programmes and their agreed models of care for patients and their families as they relate to the role• Use specialist knowledge to support and enhance general nursing practice The job description indicates the main function and responsibilities of the post and is subject to review and amendment. Statutory Registration, Professional Qualifications, Experience, etc(a) Eligible applicants will be those who on the closing date for the competition:(i) Are registered in the relevant division of the Register of Nurses & Midwivesmaintained by the Nursing and Midwifery Board of Ireland (NMBI) (BordAltranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.(ii) Have at least 5 years post registration experience (or an aggregrate of 5 yearsfulltime post registration experience) of which 2 years must be in the specialityor related area.(iii) Have the clinical, managerial and administrative capacity to properly dischargethe functions of the role.(iv) Candidates must demonstrate evidence of continuous professional development.(b) Candidates must possess the requisite knowledge and ability including a high standard ofsuitability and clinical, managerial and administrative capacity to properly discharge thefunctions of the role.2.
