Job Purpose:
The Senior Occupational Therapist will have responsibility for provision of a specialised Occupational Therapy Service for the frail population presenting to the Emergency Department as part of the multidisciplinary FIT team in the Mater Misericordiae University Hospital. The Mater FIT team is one of the largest in the country with a Geriatrician, Registrar, 2 ANP, 1 CNS, 1 pharmacist, 1 CS Dietician, 1 CS Speech and language therapist, 1 Senior Medical Social worker, 1 CS Physiotherapist, 1 Senior Physiotherapist, 1 CS Occupational therapist, 1 Senior Occupational Therapist, 1 therapy assistant. The team works closely with the wider ED team in providing expert knowledge for older person’s care including staff education sessions.
The therapist is part of the wider Care of the Older Person Occupational Therapy service which comprises the Frailty Intervention Team (FIT), the Geriatric Day Hospital, the Acute Medical Assessment Unit, the Specialist Geriatric Ward, and specialist off-site post-acute care service. There is a strong MDT focus across the services with the use of the Comprehensive Geriatric Assessment from ED throughout, with a home first ethos. The therapist will work in this clinical area encompassing the development of the Care of the Older Person Clinical Programme. The therapist will be required to work across ED, inpatient service and where applicable outpatients. Clinical input may be required into other specialty services from time to time.
Responsibilities:
1. Identify and assess the frail population presenting to ED as part of the multi-disciplinary FIT team in accordance with the mission, values, and strategic plan of the organisation.
2. Act as senior decision-maker, with recommendations regarding HSCP intervention requirements & need for hospital admission, where appropriate.
3. Determine discharge plan and appropriate follow-up arrangements with relevant services.
4. Alternate team leader role within FIT team, providing leadership and direction to the FIT team activities.
5. Participate in the management of the occupational therapy service as a member of the senior occupational therapy team in co-operation with the occupational therapy manager.
6. Analyse service demands and develop the FIT team with other HSCP members in line with evolving requirements.
7. Undertake highly skilled and specialised assessment and intervention as part of the occupational therapy process, including but not limited to:
1. Activities of daily living
2. Cognitive perception
3. Postural management and positioning
4. Sensorimotor function of the upper limb
5. Delirium
6. Management of Falls
7. Discharge Planning
